When you come in for your initial visit, please be sure to fill out all your registration forms prior to your appointment. If you need any assistance filling out the forms, please let Lisa in the front office know and we would be happy to assist you. Please bring a list of any medications you are taking. If you need antibiotics prior to dental work, please take them as prescribed. If you are not sure if you need antibiotics, please contact your doctor prior to your visit.
